Notification of Health Requirement for School Entry
The first year at school is a very important time in your child’s life. Experience has shown that the children who are not well cannot profit fully from the school program. In order to assure your child the maximum opportunity to learn he/she should be in the best possible health.
- For TK and Kindergarten entry, documentation of all immunizations and a negative PPD/Mantoux Tuberculin Skin Test or “Negative Risk” Assessment on or after the 4th birthday is required.
- For First Graders, physical examination by a physician is required within the last 18 months before entry.
- For new students in grades 2-8, a physical examination by a physician is required within the last two years before entry. The “Report of Health Examination” (PM171) is the form to be used or any form signed by an MD or nurse practitioner, and stamped with an office letterhead.
- All incoming 7th grade students are required to have TDAP Booster.
- New student transfers from out of state or country must also have a current (within two year) negative TB Risk Assessment signed by an MD. If the PPD skin test is used it must be read and documented by a health care provider to be valid.
If your child is transferring from another public or private school, those records must be received by this school on or before the first day of school.
If your child has a chronic health condition such as asthma or diabetes, please bring all instructions and information, signed by your health care provider, by the first day of school.
